PENDLE EAST PRIMARY CARE NETWORK CARE CO-ORDINATOR JOB DESCRIPTION JOB TITLE: PCN Care Co-Ordinator HOURS OF WORK: 30 hours per week PAY BAND: Equivalent to Agenda for Change band 4 RESPONSIBLE TO: Strategic Manager ACCOUNTABLE TO: Pendle East PCN CONTRACT TERM: Fixed term LOCATION: Pendle East Community Hub with outreach services to outlying GP Practices. PURPOSE OF ROLE: To provide co-ordination and navigation for people and their carers across health and care services, working closely with Social Prescribing Link Workers, Health and Well-being coaches and a range of primary care professionals within Pendle East Primary Care Network. Job summary: This is a new and developing role in primary care. The Care Coordinator will be a member of the Primary Care Network (PCN) Multi-Disciplinary Team (MDT) responsible for co-ordinating and facilitating some elements of care for people registered with our 5 local GP Practices to improve the quality of care and support they receive.
To help people to manage their needs through answering queries, making and managing appointments, and ensuring that people have good quality written or verbal information to help them make choices about their care. To support people to take up training and employment, and to access appropriate benefits where eligible. To support people to understand their level of knowledge, skills and confidence (their Activation level) when engaging with their personal health and wellbeing, including through the use of the Patient Activation Measure. 8.
To assist people to access self-management education courses, peer support or interventions that support them in their health and wellbeing and increase their activation level. To provide coordination and navigation for people and their carers across health and care services, working closely with social prescribing link workers, health and wellbeing coaches, and other primary care professionals, and support the coordination and delivery of MDTs within the PCN. 10. To work with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients, and where required and as appropriate, refer people back to other health professionals within the PCN.

Health & Safety The postholder will take responsibility for their own and others health, safety and security as defined in the companys health & safety policy, the staff handbook and the infection control policy and procedures. This will include (but will not be limited to): - Identifying the risks involved in work activities and undertaking such activities in a way that manage s those risks - Using appropriate infection control procedures, - Maintaining work areas in a tidy and safe way and free from hazards, and initiation of remedial / corrective action where needed - Actively identifying, reporting, and correcting health and safety hazards and infection hazards immediately when recognised Equality and diversity.
