**Key Responsibilities**:

- To take a leadership role in the daily management of the service, providing social practical support to service users and staff on a daily basis. Planning meetings and identifying needs & opportunities for people supported by the service..
- To formulate, implement and regularly review support plans for care with service users. Monitoring the arrangements for review meetings and supporting with these arrangements where necessary. Liaising with referrers, carers, family and other outside agencies is an integral part of this role. To evaluate the effectiveness of plans and interventions with colleagues, ensuring staff involve service users whenever possible, adhering to the risk assessment and management guidance.
- To work alongside the Rota coordinators to help support creation of weekly rotas in a person-centered manner, ensuring the needs of the people are being met.

Attend team meetings as required.

Ensure all mandatory training is up to date.

Act to ensure compliance with health and safety legislation, service specific policies and procedures.

Complete regular staff team meetings.
- Develop and maintain good working relationships with relevant agencies creating close links with local community resources.

Complete home visits with service users and families and ensuring files are in good order and condition. Ensuring support being provided is of high standard and in line with person centered planning

To undertake office-based tasks such as filing, record keeping and telephone support where required within the role.

To implement a flexible approach to the office environment and ensure all shifts are covered, using the tools provided.

To ensure all documentation/information received from support staff is acted upon appropriately and records kept up to date.

To be responsible for the monitoring/ creating of Risk Assessments and to ensure that they are in place and adhered to at all times.

To take an active role in the creation of support plans and their upkeep once an initial assessment has taken place for new services.

To actively be involved in the quality control processes and where appropriate carry out quality checks on the service delivery.
- Manage own workload ensuring that service user home visits are completed in time frame agreed and recorded to a high standard.
- Manage own workload ensuring that service user customer care phone calls are completed and recorded to a high standard.
- Manage issues, complaints within their area, staff/service users.
- Deliver MAR sheets ensuring that they are accurate and up to date.
- Providing ongoing support for staff in the form of quality control processes and informal day to day support

Ensuring that the support delivered by staff meets the standards required

Providing support in the community to service users where required.

Handling calls from staff and service users and providing advice and support

Responsibilities may be varied from time to time as agreed by post holder and Registered Manager.

**Job Types**: Full-time, Permanent

Pay: £13.25 per hour

**Experience**:

- Home care: 1 year (preferred)
- Care home: 1 year (preferred)

Licence/Certification:

- Driving Licence (required)
