About the Role

The Pendle Medical Partnership is seeking a highly skilled and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in supporting our patients and clinicians to deliver the best possible care.

Key Responsibilities
  • Process appointment requests and administrative queries via our electronic consultation system, ensuring patients are directed to the most appropriate healthcare professional.
  • Deal with patients by telephone and in person, utilizing our clinical system to provide efficient and effective care.
  • Work in accordance with our practice protocols, policies, and procedures to ensure seamless delivery of care.
  • Deal with general enquiries and make follow-up appointments to ensure patients receive the care they need.
  • Handle and process prescriptions, ensuring timely and accurate delivery.
  • Deal with requests for home visits, prioritizing patient needs and ensuring timely responses.
  • Process emails received into the practice, responding promptly and efficiently.
  • Register new patients onto our clinical system, ensuring accurate and up-to-date records.
  • Process patient changes of address on our clinical system, maintaining accurate records.
  • Respond to tasks and queries from the practice team, providing support and guidance as needed.
  • Input patient data into our clinical system, ensuring accurate and timely updates.
  • Scan patient information and documentation, maintaining accurate and secure records.
  • Advise patients of relevant charges for private services and accept and process electronic payments.
  • Deal with conflict and work with patients to ensure their needs are met in line with practice protocol.
  • Ensure the reception and waiting area is kept clean and tidy, maintaining a welcoming environment for patients.
  • Provide additional cover for annual leave and sickness absence, ensuring continuity of care.
  • Perform any other tasks allocated by managers or clinical staff, demonstrating flexibility and adaptability.
  • Open or lock up the building as required, ensuring security and safety.
  • Work occasional Saturday mornings, providing flexibility and support to the practice team.
Requirements
  • Experience of working in a busy office environment, with the ability to multitask and prioritize tasks effectively.
  • Experience of dealing with confidential information, maintaining patient confidentiality and security.
  • Experience of working in a GP practice, healthcare setting, or customer service setting that involved direct patient/customer contact.
  • Knowledge of the EMIS system, with the ability to utilize it effectively.
  • Experience of dealing with vulnerable people, demonstrating empathy and understanding.
  • Experience of working in a team setting, with the ability to collaborate and communicate effectively.
