Care Advocate Coordinator

We are seeking a highly motivated and compassionate Care Advocate Coordinator to join our team at Heather Senior Care and NDIS.

As a Care Advocate Coordinator, you will play a vital role in supporting our senior clients in navigating the My Aged Care system and developing personalized care plans that promote choice, control, and independence.

Key Responsibilities:

  • Conduct regular in-person reviews with clients to understand their current and future needs.
  • Develop and implement personalized care plans that align with client goals and preferences.
  • Work collaboratively with our team of Nurses and Carers to ensure seamless support for clients.
  • Travel to client locations using company vehicles to provide in-home support.

Requirements:

  • Current First Aid and CPR certificate.
  • Evidence of Influenza (2024 formulation) and Covid-19 vaccination.
  • Australian or New Zealand citizenship, permanent residence, or valid working visa.
  • A current and valid OPEN Australian driver's licence.

The successful candidate will receive comprehensive training on the My Aged Care package and associated processes. We offer a competitive salary of $65,000 - $75,000 per annum, depending on experience.
