Job Title: Nurse Care Coordinator

About the Role:
We are seeking a skilled and compassionate Registered Nurse to join our team in Ipswich. As a Nurse Care Coordinator, you will be responsible for providing high-quality care to our residents, working closely with colleagues to ensure their physical and emotional needs are met.

Key Responsibilities:
  • To assess, plan, implement, and evaluate individualized care plans to meet the physical, social, and emotional needs of residents.
  • To work collaboratively with other healthcare professionals to provide comprehensive care.
  • To maintain accurate records and reports, adhering to relevant policies and procedures.
  • To participate in training and development opportunities to enhance your skills and knowledge.

Requirements:
To be successful in this role, you will need:
  • A valid NMC registration as a Registered Nurse (RGN, RMN, or RNLD).
  • A minimum of 6 months' experience as a Registered Nurse.
  • Full eligibility to live and work in the UK.
