Responsibilities:
  • Collaborate with healthcare professionals to develop comprehensive care plans for individuals with complex needs
  • Conduct regular assessments to monitor progress and identify areas for improvement
  • Work closely with charity partners to provide essential services and support
Requirements:
  • Qualified Social Worker with experience working with vulnerable populations
  • Approved Mental Health Professional (AMHP) or willingness to obtain certification
  • Strong communication and interpersonal skills

Salary Information:

The annual salary for this position is between £45,432 and £61,749, inclusive of London weighting
