Complex Care Coordinator

We are seeking a highly motivated and organized individual to join our team as a Complex Care Coordinator. In this role, you will work closely with patients, carers, and healthcare professionals to develop and implement complex care plans.
About the Role
As a Complex Care Coordinator, you will be responsible for managing a caseload of patients with complex needs and developing care plans in collaboration with the MDT. You will ensure that all necessary steps are taken to provide patients with the necessary care and support, including arranging assessments, coordinating care packages, and communicating with patients, carers, and healthcare professionals.
Key Responsibilities
The key responsibilities of this role include:
- Managing a caseload of patients with complex needs and developing care plans in collaboration with the MDT
- Coordinating care packages and arranging assessments with external agencies
- Communicating with patients, carers, and healthcare professionals to ensure a smooth transition
- Escalating any issues or concerns to relevant team leaders or clinical leaders
Requirements
To be successful in this role, you will need to have:
- A NVQ 3 or Diploma Advanced Apprenticeship Level 3 Health & Social Care or equivalent
- Practical experience in a healthcare setting
- The ability to work independently and prioritize a varied workload
Estimated Salary Range
The estimated salary range for this role is £30,000 - £40,000 per annum, depending on experience.
