Carer Support Specialist

Main Purpose of the Role
As a member of the Local Solutions team, you will work closely with unpaid carers to provide them with the support and resources they need to thrive. Your role will involve empowering carers to take control of their own health and wellbeing, and connecting them with community groups and statutory services that can provide them with the support they need.
Key Responsibilities
- To support carers through the Carers Assessment process, using a variety of methods including home visits and community outreach.
- To complete Carers Support Plans with carers, using a person-centred approach that takes into account their individual needs and goals.
- To provide carers with information about the range of services available to them, and to support them in accessing these services.
- To work collaboratively with colleagues in statutory services to ensure that carers receive the support they need.
- To develop and sustain links with partnership organisations to encourage collaborative working and support unpaid carers.
Accountabilities
- To adhere to data protection legislation and data sharing agreements, and to ensure that all carers supported by the service are accurately recorded using agreed templates.
- To meet any internal or external demands for information to demonstrate compliance with targets, standards, statutory, regulatory, or other requirements.
- To maintain a high level of customer focus and to provide excellent support to all internal and external parties.
Additional Duties
- To carry out any other duties appropriate to this post, as necessary or as requested.
- To participate in professional development, using appropriate learning opportunities to improve relevant knowledge and skills.
- To ensure that all activities of the project provide equality of opportunity.
- To ensure that all policies and procedures are implemented within your working practice.
Requirements
- Experience of working in health or social care.
- Experience of supporting unpaid carers or working with people.
- Ability to work as part of a team, prioritise own workload, manage own diary, build relationships with carers and professionals, and use management databases.
- Attendance at occasional evening or weekend events.
- Technical expertise in Microsoft Office and Outlook.
- Enhanced Disclosure & Barring Check and mandatory safeguarding training as appropriate for the role.
- Travel is required, so a car owner/driver is essential for this city-wide post.
