Clinical Pharmacist Opportunities

About the Role
We are seeking an experienced Clinical Pharmacist to join our team at Peel House Medical Practice. The successful candidate will be responsible for providing high-quality medicines management services to our patients, working closely with our clinicians and healthcare professionals.
Key Responsibilities
- Undertake medication reviews, face-to-face, remote, and e-consultations, and proactively manage patients with complex polypharmacy.
- Work closely with the wider team to advise and support on medication management issues.
- Contribute to reducing medicine-related hospital admissions and re-admissions by supporting patients to get the best outcomes from their medicines.
- Implement drug withdrawals and alerts, eg MHRA, and work with the primary care professional and patients to implement NICE and other evidence-based guidelines.
- Act as a source of medicine information for the practice team and patients.
