We are seeking an experienced and dynamic primary care pharmacist to join our team. The successful candidate will work closely with our team to deliver an effective, efficient, and quality medicines management service to our patients and team.

Key Responsibilities:
  • Conduct medication reviews for housebound and care home patients as needed.
  • Reconcile medicines following hospital discharge and work with patients and community pharmacists to ensure they receive their medication post-discharge.
  • Support long-term condition reviews via face-to-face and telephone appointments.
  • Work with primary care professionals and patients to implement NICE and other evidence-based guidelines.

The ideal candidate will have a Clinical Pharmacy Diploma and 52 weeks of pre-registration training in England, Scotland, or Wales. They will also be a member of the Royal Pharmaceutical Society and have evidence of Continuing Professional Development.
