**Job Description:**

We are seeking a highly motivated and resourceful qualified Social Worker to join our team at SintChristophorus. As a Forensic Social Care Specialist, you will play a key role in supporting service users in their recovery journey within the forensic mental health in-patient setting.

The ideal candidate will be experienced in working with vulnerable individuals and have a strong understanding of the Care Programme Approach (CPA) and Multi-Agency Public Protection Arrangements (MAPPA). You will work closely with the multidisciplinary team to assert the social perspective and assess, plan, and implement patient-centred programmes of care/interventions.

You will also contribute to assessments of needs under the Care Act 2014 and Section 117 Aftercare needs, working with Responsible Local Authority and ICB partners to ensure arrangements are made for effective discharge planning. Additionally, you will identify potential barriers to discharge at the earliest possible stage and work with others to help overcome these.

As a Thames Valley Mental Health Practitioner, you will provide support to the wider ward team in regard to both children's and adult safeguarding responsibilities. You will also contribute to ongoing care planning for in-patients and work with partner organisations and the wider MDT to ensure risk is managed effectively.

The estimated salary for this position is around £50,000 - £60,000 per annum, depending on experience.
