Care Coordinator

This is a challenging and rewarding opportunity to provide complex care and support to individuals in need. As a Care Coordinator, you will be responsible for delivering high-quality care and support, working flexible shifts, including nights and weekends, and traveling between Newquay, Truro, and surrounding areas as required.
Key Responsibilities:
- To provide complex care and support to individuals with diverse needs
- To work collaboratively with colleagues and external partners to achieve optimal outcomes
- To maintain accurate records and reports, ensuring compliance with regulatory requirements
Requirements:
- A minimum of experience in complex care, preferably with a focus on case management
- A valid driver's license (ideally)
- Ability to commute to Newquay and surrounding areas
Salary and Benefits:
- £15.30 per hour, paid weekly
