Mental Health Care Coordinator

We are seeking a highly motivated Mental Health Care Coordinator to join our team at Cornwall Partnership NHS Foundation Trust. As a key member of our multidisciplinary team, you will be responsible for providing high-quality care and support to individuals with severe and enduring mental health problems.

About the Role
In this challenging yet rewarding position, you will be responsible for assessing care needs, developing and implementing personalized care plans, and collaborating with colleagues to ensure seamless delivery of services. You will also play a key role in promoting recovery-oriented practice and contributing to the development of new initiatives within the team.
The ideal candidate will have current professional registration as a Registered Nurse (RN) or equivalent, extensive experience working with mental health patients, and a proven track record of clinical leadership. You will also possess a sound working knowledge of the Mental Health Act 1983 and other relevant legislation.
