Support Services Coordinator Job Description

We are seeking a highly skilled and motivated Support Services Coordinator to join our team at Menna House, a 5-bed residential home located in a quiet rural location near Truro. About the Role:

- Coordinate support services to ensure seamless delivery of care and support
- Develop and implement individualised care plans to promote independence and well-being
- Support individuals in maintaining healthy lifestyles and managing their health needs

Salary: £25,000 - £30,000 per annum, depending on experience.
