**Job Title:** Community Support Coordinator

**Salary:** £35,000 - £40,000 per annum

We are seeking a highly motivated and experienced individual to join our team as a Community Support Coordinator. This is an exciting opportunity to work with individuals with learning disabilities and make a real difference in their lives.

The successful candidate will be responsible for leading a team of frontline staff to support individuals to achieve their personal outcomes, live the life they choose, and become an inclusive member within their community.

In this role, you will:

  • Manage several small teams across the area, providing support to a wide variety of people with a range of support needs.
  • Develop and implement robust service designs, support strategies, and risk assessments while managing individual budgets.
  • Ensure excellent organisational skills and the ability to prioritise your workload while working under pressure.
  • Communicate effectively with the people we support, staff teams, families, and external care professionals.

To be successful in this role, you will need:

  • Experience working with adults and children with learning disabilities, epilepsy, autism, and physical support needs.
  • Experience in using person-centred planning techniques and delivering excellent support practices.
  • Strong facilitation skills to encourage staff teams to take ownership and responsibility for the quality of support they deliver.
  • SQP Level 3 in Health and Social Care or equivalent.
  • Full driving licence with access to your own vehicle for business use.
