About the Role

We are seeking a highly skilled and experienced Mental Health Liaison Practitioner to join our team at Avon & Wiltshire Mental Health Partnership NHS Trust. As a key member of our Primary Care Liaison Service, you will play a vital role in providing mental health support and guidance to patients, carers, and healthcare professionals in community settings.

Key Responsibilities
  • Provide comprehensive assessments and psychoeducation to individuals referred through a range of sources.
  • Foster strong collaborative and trusting working relationships with referring agents and Primary Care Health Care Professionals.
  • Develop and implement effective care plans in collaboration with multidisciplinary teams.
  • Provide expert advice and guidance on mental health issues, including risk management and safeguarding.
  • Contribute to the development of safe and effective mental health care practice within the primary care setting.
Person Specification
  • Professional qualification in mental health equivalent to RMN, Registered Social worker or OT.
  • Current registration with a professional body and commitment to CPD.
  • Front-line experience of using advanced assessment skills, especially in determining the nature and severity of mental illness/health.
