We are seeking a highly skilled and experienced Mental Health Liaison Practitioner to join our team at Avon and Wiltshire Mental Health Partnership NHS Trust. As a key member of our Primary Care Liaison Service, you will play a vital role in providing mental health support and interventions to individuals in community settings.
Key Responsibilities- Provide comprehensive assessments and psychoeducation to individuals referred through a range of sources.
- Foster strong collaborative and trusting working relationships with referring agents and Primary Care Health Care Professionals.
- Develop and implement effective mental health care plans in collaboration with multidisciplinary teams.
- Provide evidence-based specialist advice on mental health needs and best practice interventions.
- Collaborate with service users, relatives, and carers to ensure seamless transitions into AWP provider/treatment services.
- Professional qualification in mental health equivalent to RMN, Registered Social worker or OT.
- Current registration with a professional body and commitment to CPD.
- Front-line experience of using advanced assessment skills, especially in determining the nature and severity of mental illness/health.
