We are seeking a highly skilled and experienced Mental Health Liaison Practitioner to join our team at Avon & Wiltshire Mental Health Partnership NHS Trust. As a key member of our Primary Care Liaison Service (PCLS), you will play a vital role in providing mental health support and guidance to patients, carers, and healthcare professionals in the community.
Key Responsibilities- Provide comprehensive assessments and psychoeducation to individuals referred to the service, using advanced assessment skills to determine the nature and severity of mental illness/health.
- Foster strong collaborative and trusting working relationships with referring agents and Primary Care Health Care Professionals, promoting an educative and solutions-focused approach.
- Develop and implement effective care plans, working closely with multidisciplinary teams to ensure seamless transitions into secondary mental health services.
- Provide expert advice and guidance on mental health issues, including risk management, medication management, and safeguarding.
- Contribute to the development of safe and effective mental health care practice within the primary care setting, promoting positive attitudes and collaboration between mental health staff and non-mental health professionals.
- Participate in clinical audit and service evaluation programs, ensuring high-quality care and service delivery.
- Professional qualification in mental health, equivalent to RMN, Registered Social Worker, or OT.
- Current registration with a professional body and commitment to Continuing Professional Development (CPD).
- Front-line experience of using advanced assessment skills, particularly in determining the nature and severity of mental illness/health.
- Able to articulate a detailed understanding of relevant legal frameworks/legislation, including the Mental Health Act, Mental Capacity Act, and Safeguarding.
- Demonstrates an understanding of recovery principles and the role of secondary mental health services in delivering care.
