Learning Disability Care Coordinator

Job Description
NHS National Services Scotland is recruiting for a Learning Disability Care Coordinator to work in Orkney. This full-time role, based in Kirkwall, requires the successful candidate to work closely with the multidisciplinary learning disability community health and social care team.
Key Responsibilities
- Coordinating care plans for individuals with learning disabilities and/or autism.
- Developing and maintaining relationships with primary health care teams, community groups and other stakeholders.
- Facilitating specialist learning disability support from NHS Grampian.
Requirements
- Registered Learning Disability Nurse qualification.
- Experience of positive behaviour intervention.
- A car driver with access to a vehicle.
Salary and Benefits
The salary for this position is £37,679 - £45,343 per annum, including distance islands allowance pro rata. An excellent salary and benefits package available, including pension scheme and opportunities for professional development.
