Job Description:

As a Care Coordinator, your primary responsibility will be to assess the needs of our residents and develop personalized care plans that cater to their unique requirements. You will work closely with our healthcare professionals to ensure seamless delivery of care and services.

You will also be responsible for:

  • Coordinating care activities and services to meet resident needs
  • Assessing resident needs and developing care plans
  • Collaborating with healthcare professionals to ensure high-quality care

To succeed in this role, you will need:

  • Excellent communication skills to work effectively with residents, families, and healthcare professionals
  • Strong organizational skills to coordinate care activities and services
  • Compassion and empathy to provide high-quality care and support to residents

We offer a competitive salary of £25,000 - £30,000 per annum, depending on experience, as well as a range of benefits including free meals, access to excellent training, and career development opportunities.
