Care Needs Assessor Job Opportunity

Hatched Recruitment Group is seeking a skilled Care Needs Assessor to join their team.

About the Role

In this role, you will be responsible for assessing the needs of vulnerable adults, developing comprehensive support plans to meet those needs, and working with other professionals to provide comprehensive care and support.

As a Care Needs Assessor, you will evaluate and highlight varying needs for different types of care support options, traditional care, reablement or supported living.

Key Responsibilities
  • Carry out social care and joint health assessments
  • Devise comprehensive support plans to meet identified needs
  • Evaluate and highlight varying needs for different types of care support options
  • Participate in care discussions and multidisciplinary assessments regarding customers

We are looking for someone with experience of working in social care or a similar field, a strong understanding of social care principles and practices, and excellent communication and interpersonal skills.

The salary for this role is approximately £22,500-£24,500 per annum, depending on experience.
