Job Title: Care Needs Assessor

Hatched Recruitment Group is seeking a highly motivated and organized Care Needs Assessor to join our team. As a Care Needs Assessor, you will be responsible for assessing the needs of vulnerable adults, providing packages of care, and reviewing and monitoring Community Care Packages.

About the Role:

  • You will carry out comprehensive assessments of vulnerable adults to identify their needs.
  • You will develop and implement packages of care in line with assessed need, within available resources, and according to relevant social care criteria.
  • You will work closely with clients, families, and other professionals to ensure that the needs of vulnerable adults are met.

Requirements:

  • Experience of working in Social Care Services or similar organisations, such as the NHS or Voluntary Sector.
  • Experience of assessing the needs of people, devising reablement/outcomes/goal setting for support planning.
  • Experience of working with vulnerable adults with learning disabilities in a social care context.

What We Offer:

  • A competitive salary of £40,000 - £48,000 per annum, depending on experience.
