Job Description

Main Responsibilities:

  • Conduct comprehensive assessments of children's healthcare needs.
  • Develop and implement care plans tailored to each child's unique requirements.
  • Support and advise social workers on Continuing Care assessments and procedures.

Essential Requirements:

  • Relevant qualifications and experience in healthcare or a related field.
  • Exceptional communication and interpersonal skills.
  • Able to work effectively in a dynamic environment.
