About the Job

The Assertive Care Coordinator will work as part of our Assertive Outreach Team, coordinating care for individuals with severe and enduring mental health challenges within the Community Mental Health Service.


Key responsibilities include conducting comprehensive assessments, developing care plans, and providing evidence-based interventions. The ideal candidate will have experience in working closely with Service Users, the Voluntary Sector, and Community Enterprises to deliver integrated, person-centered care.


A strong understanding of mental health legislation, policies, and procedures is essential for this role, along with the ability to work effectively within established professional boundaries.
