Mental Health Services Coordinator Role

An exciting opportunity has arisen for a highly motivated and experienced Mental Health Practitioner to join our innovative team at Somerset NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a pivotal role in delivering exceptional mental health care to individuals with severe and enduring mental health challenges.

In this demanding yet fulfilling position, you will be responsible for developing and implementing personalized care plans, ensuring seamless care coordination and applying creative risk assessment and management strategies to guarantee safe and effective patient care.

Main Duties
  • Personalized care planning: Work independently to develop and implement tailored care plans, focusing on the individual's unique mental health needs and goals.
  • Care coordination: Collaborate with colleagues to ensure efficient caseload management, providing comprehensive support to individuals with varying mental health challenges.
  • Risk management: Implement effective risk assessment and management strategies to safeguard patient safety and well-being.

Person Specification
  • Qualifications: RMN, Social Work or Occupational Therapy Qualification, Current registration with relevant professional body.
  • Experience: Significant post-registration experience in community mental health settings, including knowledge of working with individuals experiencing mental health problems.

Estimated Salary: £46,000 - £66,000 per annum, depending on experience.
