About the Role:
We are seeking a highly motivated and organized individual to join our team as a Care Coordination Specialist. This is an exciting opportunity to work in a dynamic environment where you will have the chance to make a real difference in the lives of patients.
Key Responsibilities:
- Coordinate the delivery of personalized health budgets and care planning to patients.
- Work closely with care providers to ensure seamless delivery of services.
- Develop and maintain effective relationships with patients, families, and healthcare professionals.
- Ensure that all interventions are patient-centered, delivered compassionately, and with dignity.
Requirements:
- 5 GCSEs grades C or above, including Maths and English, or equivalent qualification.
- Educated to NVQ 3 level in a relevant subject or equivalent experience.
- Excellent communication and organizational skills.
- Awareness of equality and valuing diversity principles.
- Understanding of Confidentiality, Data Protection, and GDPR.
What We Offer:
- An estimated salary of £25,000 - £35,000 per annum.
