Community Outreach Worker

The Community Outreach Worker will work as part of our Assertive Outreach Team, delivering high-quality mental health services to individuals with severe and enduring mental health challenges within the Community Mental Health Service.
Key responsibilities include conducting comprehensive assessments, developing care plans, and providing psychosocial interventions. The ideal candidate will have experience in working closely with Service Users, the Voluntary Sector, and Community Enterprises to deliver integrated, person-centered care.
A strong understanding of mental health legislation, policies, and procedures is essential for this role, along with the ability to work effectively within established professional boundaries.

About the RoleThe successful candidate will receive regular supervision to support ongoing skill development and competence, as well as opportunities for professional growth and advancement within the Trust.

For the right candidate, we offer a competitive salary of £35,000 - £45,000 per annum, depending on experience, plus a range of benefits including flexible working, an NHS pension scheme, and generous annual leave allowance.
