NHS Continuing Healthcare Support Worker

Job Summary:

This is an exciting opportunity to work in a dynamic environment where you will have the chance to make a real difference in the lives of patients. As an NHS Continuing Healthcare Support Worker, you will play a key role in supporting the Continuing Healthcare Service, working within the remit of the current National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care.

Key Responsibilities:

  • Support the Continuing Healthcare team to ensure National Framework compliant processes and local processes are completed in a timely manner.
  • Communicate information, risks, issues, and dependencies, including briefings and reports.
  • Maintain up-to-date knowledge of legislation, policies, and guidance.
  • Actively support the Continuing Healthcare team in working towards goals and objectives.

Requirements:

  • 5 GCSEs grades C or above, including Maths and English, or equivalent qualification.
  • Educated to NVQ 3 level in a relevant subject or equivalent experience.
  • Excellent communication and organizational skills.
  • Awareness of equality and valuing diversity principles.
  • Understanding of Confidentiality, Data Protection, and GDPR.

What We Offer:
