Job Description:

In this challenging yet rewarding role, you will be responsible for managing a caseload of vulnerable adults, conducting assessments and reviews of their mental health and wellbeing. You will also acknowledge information from various sources to identify and deliver suitable services and support.

Key Responsibilities:

  • Provide one-to-one support for adults with mental health conditions within their own homes and local communities.
  • Manage a caseload of vulnerable adults, conducting assessments and reviews of their mental health and wellbeing.

  • Ensure service user involvement by making their voice central to all aspects of their care and support.
  • Address housing management and organisational issues affecting service users' mental health or wellbeing.
  • Collaborate with the supportive and dedicated team to deliver high-quality care and support services.

Requirements:

  • A valid driving license and access to your own vehicle, with business insurance.
  • Experience in supporting adults with mental health conditions is preferred, but we will consider applicants with a strong commitment to making a difference.
  • Strong communication and organisational skills, with the ability to assess and review complex information.

Estimated Salary: £25,584 per annum (based on national average salaries for similar roles in Cardiff).
