Job Summary

We are seeking an experienced Hospital Pharmacy Technician to join our team, providing expert advice and support on medicines optimisation and managing care pathways for community service users. The successful candidate will have excellent interpersonal skills, IT proficiency and be registered with the GPhC.

Main Responsibilities

  1. Develop and deliver comprehensive medicines optimisation services to wards and clinics.
  2. Provide expert advice on safe prescribing, handling and administration of medicines.
  3. Work closely with the Medicines Management Technical team at St George's hospital.
  4. Rotate between hospital wards and community services.
  5. Perform medicines reconciliation on admission to hospital and on discharge as necessary.
  6. Provide medication counselling to promote self-management and independence with medication.
  7. Ensure seamless pathway from in-patient to out-patient.
  8. Promote safe, legal, rational and economic use of medicines by all users of pharmacy service.
