Rehabilitation Case Manager

Anglia Case Management is a leading provider of domiciliary care and rehabilitation services, registered with the Care Quality Commission and regularly inspected by the CQC. We have been judged Outstanding since 2017.
We are seeking an experienced Rehabilitation Case Manager to join our team in Norwich and surrounding areas. As a key member of our team, you will work with adults and children who have ongoing needs, supporting them to improve their quality of life and maximise their potential.
Key Responsibilities:
- Advocate for clients' needs, building strong relationships with families, deputies, and litigation solicitors.
- Develop and implement individualised support and rehabilitation plans, setting specific goals and timeframes.
- Recruit, select, and manage a team of care/support workers, ensuring the smooth operation of services.
- Monitor and review case management plans, making amendments as needed to reflect clients' changing needs.
- Provide flexible working patterns to support teams, including out-of-hours, weekends, and public holidays.
- Compile risk assessments and identify strategies to manage safety risks.
Requirements:
- UK professional health qualification, requiring registration with HCPC, NMC, or Social Worker.
- Full driving licence and access to a car.
- Previous experience as a Case Manager, supporting adults or children.
- Thorough knowledge of CQC fundamental standards and key lines of enquiry.
- Working knowledge of the Mental Capacity Act, Deprivation of Liberty Safeguards, and Safeguarding frameworks.
- Ability to address employee issues in a fair and consistent manner.
- Strong organisational, time management, and planning skills.
