About the Role
We are seeking a highly skilled and dedicated Stroke Rehabilitation Nurse to join our team at Norfolk Community Health and Care NHS Trust. As a Stroke Rehabilitation Nurse, you will play a vital role in providing high-quality care to patients who have suffered a stroke.
Key Responsibilities
- Provide nursing care to patients with complex needs, including those with cognitive, emotional, and physical impairments.
- Work collaboratively with the multidisciplinary team to develop and implement individualized care plans.
- Conduct assessments and provide interventions to promote patient independence and rehabilitation.
- Communicate effectively with patients, families, and carers to ensure their needs are met.
- Participate in multidisciplinary meetings and contribute to the development of care plans.
- Supervise and support junior staff members to ensure high-quality care is provided.
Requirements
- Current NMC registration as a Registered Nurse.
- Varied clinical experience, including experience of working with patients with complex needs.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Ability to prioritize workload and manage time effectively.
