About the Role

We are seeking a highly skilled and compassionate Mental Health Practitioner to join our Crisis Resolution Home Treatment Team in Slough. As a key member of our team, you will play a vital role in delivering high-quality care to individuals experiencing mental health crises.

Key Responsibilities
  • Conduct thorough assessments and develop individualized care plans to support patients in achieving their recovery goals.
  • Deliver evidence-based treatments and interventions to patients, including those with complex mental health needs.
  • Work collaboratively with multidisciplinary teams to ensure seamless care coordination and effective risk management.
  • Provide brief, follow-up services to patients to support their ongoing recovery and well-being.
Requirements
  • Registered Mental Health Nurse or Social Worker with a strong understanding of the acute care pathway.
  • Experience of assessment and case formulation, with a proven ability to manage risk and prioritize patient safety.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced, dynamic environment.
  • Ability to travel to multiple sites and work flexibly to meet the needs of the service.
