About the Role

We are seeking a highly skilled and experienced Mental Health Practitioner to join our Crisis Resolution Home Treatment Team in Slough. As a key member of our team, you will play a vital role in delivering high-quality care to our patients and providing an alternative to traditional mental health admissions.

In this role, you will be responsible for conducting thorough assessments, developing and implementing individualized care plans, and delivering evidence-based treatments to patients referred from community mental health teams, the psych liaison service, or inpatient wards.

You will work closely with a multidisciplinary team to oversee care delivery, manage risks, and evaluate care outcomes, prioritizing the recovery of each service user. Your ability to take calculated positive risks, align with the Trust Policy on Risk Assessment and Management, and work effectively in a pressurized environment will be essential.

Key Responsibilities
  • Develop, implement, and evaluate individualized care plans for patients with complex mental health needs.
  • Conduct thorough assessments and deliver evidence-based treatments to patients referred from community mental health teams, the psych liaison service, or inpatient wards.
  • Work closely with a multidisciplinary team to oversee care delivery, manage risks, and evaluate care outcomes.
  • Take calculated positive risks and align with the Trust Policy on Risk Assessment and Management.
Requirements
  • Registered Mental Health Nurse or Social Worker with a degree in health/social care or equivalent qualification/experience.
  • Substantial experience of managing a busy caseload or service users with a range of mental health conditions.
  • Experience of working effectively in a multi-disciplinary team and multi-agency environment.
  • Excellent knowledge and understanding of the health and social care systems and operating frameworks, clinical observations, and the Mental Health Act.
