Mental Health Practitioner

We are seeking a highly skilled and compassionate Adult Mental Health Practitioner to join our innovative and dynamic team at Berkshire Healthcare NHS Foundation Trust. As a key member of our Early Intervention in Psychosis Service, you will play a vital role in supporting individuals experiencing a first episode of psychosis or an at-risk mental state.
Key Responsibilities
As a Care Coordinator, you will work collaboratively with clients and their families to develop a relapse management plan, ensuring a comprehensive and holistic approach to their care. You will also undertake comprehensive assessments of health and social needs, implementing a plan of care and providing NICE-recommended interventions. Additionally, you will work closely with other stakeholders to ensure clients receive a seamless and coordinated service.
Requirements
To be successful in this role, you will require a diploma or degree-level professional qualification, registered for practice through NMC or HCPC. You will also need a thorough understanding of psychosis and NICE-recommended interventions for this client group. Experience as a care coordinator is essential, and you will be able to demonstrate evidence of continuous professional development to a high standard. You will also need to be able to travel to multiple sites and possess basic computer literacy and the ability to use electronic systems.
