About the Role

We are seeking a dedicated and compassionate Support Practitioner to join our team at Cornerstone Community Care. As a Support Practitioner, you will play a vital role in providing high-quality care and support to an individual with complex needs.

Key Responsibilities
  • Assist with daily living tasks, including personal care, meal preparation, and household chores.
  • Support the individual with mobility and transportation, including driving their mobility car.
  • Develop and maintain a positive relationship with the individual, promoting their independence and well-being.
  • Work collaboratively with the care team to ensure seamless care and support.
  • Participate in training and development opportunities to enhance your skills and knowledge.
Requirements
  • Good communication and interpersonal skills.
  • A caring and compassionate approach to care and support.
  • Ability to work flexibly, including evenings, weekends, and waking nights.
  • Full UK manual driving license with a minimum of one year's experience.
  • SVQ in Social Services and Healthcare (or willingness to work towards it).
