An exciting opportunity has arisen for a skilled Clinical Practitioner to join University Hospitals Coventry and Warwickshire (UHCW) NHS Trust as part of the One Coventry Integrated Team.

About the Role:

This is an autonomous role that requires a high standard of patient care, utilising advanced clinical skills and in-depth theoretical knowledge. The successful candidate will work independently to deliver exceptional patient outcomes within our Urgent Emergency Care pathways.

Key Responsibilities:

  • Deliver high-quality patient care as an autonomous practitioner, leveraging advanced clinical skills and theoretical expertise.
  • Effectively manage complex caseloads, providing specialist assessment and interventions to patients in both stable and acute phases of their condition.
  • Collaborate with multidisciplinary teams to develop comprehensive treatment plans, addressing patients' physical, psychosocial, behavioural, perceptual, cognitive, sensory, and environmental needs.
  • Implement evidence-based practice, signposting and referring patients to appropriate health, social, and third-sector services to reduce unnecessary attendances and admissions at A&E.
  • Foster collaborative relationships with colleagues to support staff through change management, using influencing skills to drive practice improvements.
  • Evaluate and refine practice through clinical audit and formal evaluation processes.
  • Contribute to 7-day working rotas to ensure delivery of key services, supporting the development of competencies as required.
  • Maintain accurate clinical records, adhering to trust policies, procedures, and guidelines.

Salary Information:

The estimated annual salary for this role is £60,000, based on national averages and market trends.
