Job Overview

We are seeking a highly skilled Band 5 Practitioner to join our team as a Community Health Practitioner. This is an exciting opportunity to work in a dynamic environment, providing high-quality care to patients in their homes and community settings.

The successful candidate will be responsible for initiating effective history taking, clinical assessment, diagnosis, and implementation of care interventions for patients presenting with undifferentiated and undiagnosed problems.

The role requires utilizing highly specialist knowledge and clinical reasoning to plan and provide clinical intervention in consultation with other colleagues in the multidisciplinary team.

Main Responsibilities
  • Initiate effective history taking, clinical assessment, diagnosis, and implementation of care interventions
