Join Our Multidisciplinary Team as a Community Health Practitioner

University Hospitals Coventry and Warwickshire NHS Trust is a leading healthcare provider in the UK, known for its commitment to excellence and patient-centered care. We are seeking a highly skilled Community Health Practitioner to join our multidisciplinary team and contribute to delivering high-quality patient care.

Key Responsibilities:

  1. To provide comprehensive holistic assessment and care planning for patients referred to our Integrated Single Point of Access (ISPA).
  2. To develop and implement individualized care plans in collaboration with patients, families, and other healthcare professionals.

Requirements:

  • Degree in Nursing or equivalent qualification.
  • Post-registration experience as a nurse, preferably with experience in community health.
  • Excellent communication and interpersonal skills.
  • Ability to work without direct supervision and to solve problems.

The salary for this role is approximately £50,000 - £70,000 per annum, depending on experience. This is a full-time position working between 8am-8pm, 7 days a week. You will be required to work shifts within those times and days.
