About the Role

We are seeking a highly motivated and compassionate Community Health Navigator to join our team. As a key member of our primary care network, you will play a vital role in empowering individuals to take control of their health and wellbeing.

Your primary responsibility will be to support patients in adopting a holistic approach to their health, connecting them with community groups and statutory services for practical and emotional support. You will work collaboratively with local agencies to raise awareness of social prescribing and its benefits, encouraging referrals and partnership working to reduce pressure on statutory services.

As a Community Health Navigator, you will be responsible for:

  • Building relationships with key staff in GP practices within the local Primary Care Network (PCN)
  • Developing strong links with all local agencies to encourage referrals and ensure they have the confidence to make appropriate referrals
  • Working in partnership with local agencies to raise awareness of social prescribing and its benefits
  • Seeking regular feedback about the quality of service and impact of social prescribing on referral agencies
  • Encouraging self-referrals and connecting with all local communities, particularly those that statutory agencies may find hard to reach
  • Helping people maintain or regain independence through living skills, adaptations, enablement approaches, and simple safeguards
  • Facilitating the introduction of people to community groups, activities, and statutory services, ensuring they are comfortable
  • Assisting people who may be eligible for a personal health budget in exploring this option as a way of providing funded, personalized support to be independent
What We Offer

We offer a competitive salary of £28,000 - £35,000 per annum, depending on experience, and a range of benefits including professional development opportunities, flexible working hours, and a supportive team environment.
