Community Health Specialist

This position as a Community Health Specialist focuses on providing preventive care and wrap-around services within the community to prevent hospital admissions. You will work collaboratively with a wide range of referral streams, including hospital, primary care, community referrals, pre-hospital, and the ambulance service.
Your primary responsibility will be to conduct comprehensive risk assessments, implementing safe and clinically appropriate discharge planning for patients. In addition, you will play a pivotal role in enhancing initial patient assessments using the Physical Health Assessment qualification, thereby contributing to the delivery of high-quality patient care.
