Advanced Clinical Practitioner

University Hospitals Coventry and Warwickshire NHS Trust, a leading teaching hospital in the UK, seeks a highly skilled Advanced Clinical Practitioner to join their community care team. This exciting opportunity offers a competitive salary of £44,414 - £61,609, depending on experience.
The successful applicant will work as part of a multidisciplinary team to reduce unnecessary hospital admissions and contribute to effective admission avoidance. Using high-level health assessment skills, they will identify patients with complex healthcare needs and provide targeted interventions to improve patient outcomes.
This is a fixed-term 12-month contract to cover maternity leave. The ideal candidate will have excellent clinical reasoning skills, be able to plan and provide clinical interventions in consultation with other colleagues, and work effectively in a range of settings.
Main Responsibilities:
- Initiate effective history taking, clinical assessment, diagnosis, and implementation of care interventions for patients with undifferentiated and undiagnosed problems.
- Utilize specialist knowledge and clinical reasoning to plan and provide clinical interventions in consultation with other colleagues.
- Signpost, refer, and support access to health, social, and third sector services to reduce inappropriate attendances and admissions at A&E.
