About the Role
We are seeking an experienced and motivated Care Team Coordinator to join our team at Winchcombe Place care home in Newbury. As a key member of our leadership team, you will be responsible for leading and supervising your shift, ensuring that our residents receive the highest level of care.
Key Responsibilities:
- Leading and supervising your shift, ensuring seamless operation of care delivery
- Case managing residents and liaising with families, healthcare professionals, and other stakeholders to create and deliver best care plans
- Administering and recording medication, maintaining accurate records and following procedures
- Carrying out risk assessments, identifying potential risks and implementing measures to mitigate them
- Working alongside care assistants to support residents with daily needs, promoting independence and activity
Requirements:
- NVQ Level 3 in Health and Social Care or equivalent desirable
- Proven ability to lead and supervise others, delegating tasks effectively and providing guidance
- Good understanding of care planning processes, prioritizing resident needs and delivering tailored care
- Experience in administering medication, or willingness to undertake training
- Excellent communication and interpersonal skills, working collaboratively with colleagues and external partners
