Complex Care Team Coordinator

Job Summary:
We are seeking a highly skilled and experienced Registered General Nurse to lead our care team in providing exceptional support to a young lady with complex care and medical needs. As a team lead, you will be responsible for overseeing the delivery of high-quality care and ensuring the well-being of our client.
Key Responsibilities:
- Lead a team of caregivers in providing 2:1 support to our client, ensuring her physical, emotional, and social needs are met.
- Develop and implement care plans in collaboration with our client's case manager and other healthcare professionals.
- Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal outcomes.
- Collaborate with our client's family and other stakeholders to ensure seamless care coordination.
- Provide training and guidance to caregivers to ensure they have the necessary skills and knowledge to provide high-quality care.
- Contribute to the development of policies and procedures to ensure compliance with regulatory requirements and best practices.
Requirements:
- Registered General Nurse with managerial/lead experience.
- Experience supporting children with complex medical care needs.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong problem-solving and critical thinking skills.
