We are seeking a highly skilled and motivated Clinical Case Manager to join our team at Childrens Complex Care. As a key member of our healthcare team, you will be responsible for managing a caseload of children and young people with complex healthcare needs.
Key Responsibilities- Clinical Case Management: Safely manage a caseload of children and young people with complex healthcare needs in the community setting.
- Assessment and Planning: Complete comprehensive assessments and develop bespoke care delivery plans to meet the unique needs of each child or young person.
- Clinical Governance and Risk Management: Ensure that all aspects of care delivery are in accordance with best practice and regulatory requirements.
- Education and Training: Provide classroom-based training and support to community staff to ensure safe and effective care delivery.
- Staff Development and Support: Educate, support, and complete competency assessments for non-registered healthcare staff on clinical delegated tasks.
- Collaboration and Communication: Work effectively with internal and external stakeholders, including ICBs and external multidisciplinary teams, to ensure seamless care delivery.
- Clinical Accountability: Ensure that all reasonable measures are in place to deliver safe and effective evidence-based care to all children and young people.
To be successful in this role, you will need to be a UK registered nurse (RSCN or RGN) with strong clinical, management, and communication skills. You will also need to be committed to quality and high standards of care and service delivery.
