Job Title: Mental Health Practitioner - Assertive Outreach Service

We are seeking a skilled Mental Health Practitioner to join our Assertive Outreach Service team. As a key member of our team, you will be responsible for assessing, planning, delivering, and evaluating care for service users referred to our service.

Main Duties and Responsibilities:

  • Assess and manage risk for service users with complex mental health needs.
  • Develop and implement comprehensive care packages for service users, working within a psychosocial and family-centered framework.
  • Collaborate with other agencies and disciplines to provide holistic support to service users.
  • Provide lead professional support for service users with complex needs, developing therapeutic relationships and administering medication as required.
  • Communicate effectively with service users, carers, and other stakeholders to provide support and information.

Requirements:

  • RMN or Registered Mental Health Nurse qualification.

  • Valid driving license and access to a car.
