Palliative Care Team Lead

Role Overview:
As a Palliative Care Team Lead, you will be responsible for managing your own daily workload of patients, visiting them in their homes, clinics, or occasionally on wards. You will work closely with a multidisciplinary team, including occupational therapists, physiotherapists, doctors, clinical psychologists, and nurse specialists.

Responsibilities:
* Provide specialist Physiotherapy/ Occupational Therapy assessments and interventions for patients living with breathlessness and associated anxiety and fatigue.
* Develop and maintain effective working relationships with healthcare professionals in primary and secondary care.
* Identify opportunities for education and training and develop resources to support these activities.
* Participate in evidence-based practice, audit, and research projects related to palliative care and respiratory conditions.

Requirements:
* Diploma/Degree in Occupational Therapy/Physiotherapy.
* State Registration.
* Evidence of relevant post-graduate courses/CPD.
* At least 3 years of experience in Palliative Care, Oncology, Rheumatology, Respiratory, Elderly Care, Neurology, and Community/home visits.

Working Hours:
This full-time permanent role offers flexible working arrangements, including part-time working, job-share, term-time working, and flexible start and finish times.
