Job Summary

We are seeking a highly skilled and experienced Clinical Discharge Lead to join our team at University Hospitals Dorset NHS Foundation Trust. As a key member of our Discharge Service, you will play a vital role in ensuring the safe and timely discharge of patients from our acute hospitals.

Key Responsibilities
  • Develop and implement effective discharge planning strategies to meet the needs of patients and their families.
  • Lead a multidisciplinary team to coordinate discharge planning and ensure seamless transitions of care.
  • Collaborate with internal and external stakeholders to identify and address barriers to discharge.
  • Monitor and evaluate the effectiveness of discharge planning processes and make recommendations for improvement.
  • Provide education and training to staff on discharge planning best practices.
Requirements
  • Registered Nurse or Registered Healthcare Professional with relevant experience in an acute hospital setting.
  • Proven track record of leadership and influencing skills with the ability to enthuse, motivate and involve individuals and teams.
  • Effective communicator able to communicate highly complex and sensitive information where there may be barriers to understanding.
  • Able to work independently, be self-motivated and prioritize own caseload and support others with management of their caseload.
