Job Title: Clinical Discharge Lead

We are seeking a highly skilled and experienced Clinical Discharge Lead to join our team at University Hospitals Dorset NHS Foundation Trust. As a key member of our Discharge Service, you will play a vital role in ensuring the safe and timely discharge of patients from our acute hospitals.

Job Summary

The successful candidate will have a proven track record of leadership and influencing skills, with the ability to enthuse, motivate and involve individuals and teams. You will be responsible for coordinating referrals to the Transfer of Care Hub (TOCH) and working with internal and external partners to achieve a safe and timely discharge.

Key Responsibilities
  • Develop and implement effective discharge planning strategies to ensure the safe and timely discharge of patients
  • Coordinate referrals to the TOCH and work with internal and external partners to achieve a safe and timely discharge
  • Lead and manage a multidisciplinary team to ensure the effective delivery of discharge services
  • Develop and maintain relationships with external community service providers, local authorities and voluntary sector organizations
  • Monitor and evaluate the effectiveness of discharge services and make recommendations for improvement
Requirements
  • Registered Nurse or Registered Healthcare Professional (AHP) relevant to speciality
  • Experience of leadership and influencing skills with the ability to enthuse, motivate and involve individuals and teams
  • Effective communicator able to communicate highly complex and sensitive information where there may be barriers to understanding
  • Able to work independently, be self-motivated and prioritize own caseload and support others with management of their caseload
