Job Title: Clinical Discharge Lead

We are seeking an experienced and skilled Clinical Discharge Lead to join our team at University Hospitals Dorset NHS Foundation Trust. Key Responsibilities:
  • Develop and implement discharge planning strategies to improve patient outcomes and reduce hospital length of stay
  • Lead a multidisciplinary team to coordinate discharge planning, including nurses, therapists, and administrative staff
  • Collaborate with internal and external partners to ensure seamless transitions of care
